After scheduling your appointment, you will receive an email instructing you to complete intake paperwork and consent forms. It is crucial that these forms are completed at least 24 hours before your appointment to avoid cancellation. The initial appointment is $150, unless you are using insurance. Please note that if you are approved for our ketamine treatment program, the $150 from the initial appointment will be credited to your membership plan, which brings it down to $200. As stated above, you can utilize your insurance to cover most of the initial consultation cost.
